Cycles Of Change
Please read the following before attending the clinic.
Appointments should be made online, by telephone 01509 621122 or e-mailing info@cyclesofchange.com leaving a current mobile phone number so that we can contact you easily.
Please read the following. Should you answer YES to any of the questions at any time prior to your appointment, please contact us to arrange an alternative date and time. In the last 14 days, have you, or anyone in your household:
Please also contact us if you, or someone you live with, are classed as clinically vulnerable.
We have taken the necessary government mandatory steps of conducting a risk assessment and instituting new social distancing, hygiene, hand-washing and PPE procedures in the practice to minimise the risk of Covid-19 transmission. Please be aware that in the course of the consultation, your practitioner will have to have non-socially distanced contact with you to perform the treatment, hence while they will meet very high standards of infection protection control, it is impossible to completely eliminate risk. Please let me know that you understand this and are happy to undertake treatment.
General information
In the eventuality that your practitioner displays any symptoms of COVID-19 within 48 hours of having close contact with you during the appointment and then later test positive, we are obligated under law to provide your name, phone number or email and the date and time of your visit to the test and trace service. Please note that by attending the appointment you give consent for this. To support NHS Test and Trace (part of the Department for Health and Social Care) in England, we have been mandated by law to collect and keep a limited record of patients and visitors who come onto the premises, for the purpose of contact tracing. By doing this, and by sharing these records with NHS Test and Trace where requested, we can help to identify people who may have been exposed to the coronavirus. This information will be taken from our diary entries. We also have a track and trace Q code to scan on arrival if you wish to do so and all details of data collection can be viewed as a poster on arrival.
This is a lot of information to digest, but we want to reassure you that you will still receive our full presence to hear your story. You will be treated as always with kindness and compassion in our sacred space.
We look forward to seeing you soon.
