Patient Information

Please read the following before attending the clinic.

Appointments should be made online, by telephone 01509 621122 or e-mailing info@cyclesofchange.com leaving a current mobile phone number so that we can contact you easily.

Please read the following. Should you answer YES to any of the questions at any time prior to your appointment, please contact us to arrange an alternative date and time. In the last 14 days, have you, or anyone in your household:

  • Had a high temperature, new continuous cough, or loss of sense of smell or taste.
  • Had headache, runny nose or other symptoms that could be attributed to the new COVID-19 variant?
  • Had close contact with anyone who has exhibited any of the above symptoms?
  • Had close contact with anyone with a confirmed COVID-19 diagnosis?
  • Recently travelled abroad and/or been instructed by the government to quarantine?
  • Been contacted by the government or NHS and told to self-isolate for any reason?

Please also contact us if you, or someone you live with, are classed as clinically vulnerable.

We have taken the necessary government mandatory steps of conducting a risk assessment and instituting new social distancing, hygiene, hand-washing and PPE procedures in the practice to minimise the risk of Covid-19 transmission. Please be aware that in the course of the consultation, your practitioner will have to have non-socially distanced contact with you to perform the treatment, hence while they will meet very high standards of infection protection control, it is impossible to completely eliminate risk. Please let me know that you understand this and are happy to undertake treatment.

General information

  • If at all possible, come alone or leave partners or carers to wait in the car.
  • Please keep personal belongings to a minimum. Unfortunately we are at present unable to provide drinks.
  • When you arrive, please wait in your car until you receive a text or call from your practitioner.
  • Social distancing of 2m where possible must be adhered to until the treatment commences. All arrivals and departures have been carefully spaced to avoid crossing and we are not using the reception area as a waiting room.
  • Whilst government rules and guidelines, changed recently, in order to protect all patients, ourselves and the wider community, we are requesting all patients to wear a mask before you enter the clinic and during your treatment (unless exempt). If you do not have access to one, we do have some available. We will be wearing a mask and scrubs.
  • You will be asked to hand sanitise and answer questions to exclude symptoms of Covid-19 on arrival.
  • We are unable to provide blankets or soft coverings during treatment but please feel free to bring your own with you.
  • Please wear loose tops and bottoms that can be rolled up.
  • Ideally please pay for your treatment via bank transfer before your treatment. (Cycles of Change Acupuncture Sort code 09-01-51 Account no. 94083187). Card payment is still available.

In the eventuality that your practitioner displays any symptoms of COVID-19 within 48 hours of having close contact with you during the appointment and then later test positive, we are obligated under law to provide your name, phone number or email and the date and time of your visit to the test and trace service. Please note that by attending the appointment you give consent for this. To support NHS Test and Trace (part of the Department for Health and Social Care) in England, we have been mandated by law to collect and keep a limited record of patients and visitors who come onto the premises, for the purpose of contact tracing. By doing this, and by sharing these records with NHS Test and Trace where requested, we can help to identify people who may have been exposed to the coronavirus. This information will be taken from our diary entries. We also have a track and trace Q code to scan on arrival if you wish to do so and all details of data collection can be viewed as a poster on arrival.

This is a lot of information to digest, but we want to reassure you that you will still receive our full presence to hear your story. You will be treated as always with kindness and compassion in our sacred space.

We look forward to seeing you soon.
